Referrals
Adults can self-refer to the service by completing this form. Referrals may also be made through a GP or another professional, in which case we will contact you to arrange an assessment. When the assessment appointment is confirmed, you will receive an information sheet about the service.
Children and Young People under the age of 18 can be referred to the service by completing this form. Referrals may also be made through a GP or another professional, in which case we will contact a named contact, such as parent or guardian, to arrange an assessment.
