(T.), 3 November 1914; attached 2/1st South Midland Mounted Field Ambulance; 3rd South Midland Field Ambulance Served-Home, 1914-16; France, Flanders, Italy, July 1916-November 1918. Final rank, Captain. Mentioned, May 1918.
