Biography: Son of George H. Cruickshank; born Leith, 22 March 1888. M.B., 1910; M.D. (Hons.), 1920. Lieutenant, R.A.M.C, 23 February 1915 ; attached to 28th Field Ambulance, Medical Officer, 11th Royal West Kent Regiment, 41st Battalion Machine Gun Corps, 18th King's Royal Rifle Corps, 11th Royal West Surrey Regiment. Served-Home, 1915 ; France, Italy, Cologne, 1916-19. Final rank, Captain.
