Ian Proctor is a 63 year old man who consults his GP with cough productive of grey sputum in the mornings and mild exertional breathlessness when walking his dog.
These symptoms have been present for around 6 months and he thinks they are slowly becoming worse. Mr Proctor feels that in the winter, "colds" go down into his chest and frequently make him wheeze.
He lives at home with his wife and stopped working as a refuse collector several years earlier. He started smoking at the age of 14 and still smokes around 10 cigarettes each day.
His wife is well, but she also smokes. His general health is good other than hypertension which is well controlled with medication.
