Microwave Endometrial Ablation v Thermal Balloon Ablation

Microwave Endometrial Ablation versus Thermal Balloon Endometrial Ablation: a randomised comparison of treatment in the post menstrual phase: clinical outcomes, patient acceptability and cost

Endometrial ablation is the process whereby the lining of the womb is destroyed in an attempt to reduce heavy periods and its associated problems. It is a proven conservative alternative to hysterectomy. MEA and TBEA are second generation endometrial ablation techniques which have been adequately assessed and recommended by the National Institute For Health and Clinical Excellence (NICE). The uptake of second generation endometrial ablative techniques have, however, been largely driven by the manufacturing companies rather than by good quality research. This current study was a randomised controlled trial including an economic evaluation comparing the costs to the NHS and patients over a 12 month period and QALY elicited from response to the EQ-5D for the alternative treatments.

Outcome and Translation

With a twelve month time horizon TBEA is not likely to be considered cost-effective when compared with MEA. This conclusion could change in light of further long term data on quality of life, failure rates and costs, which are currently being collected.
