Paramedic Teamwork and Safety Questionnaire

My name is Nina Pagon and I’m a Psychology student at the University of Aberdeen. This survey is being run as part of my undergraduate Level 4 thesis research project supervised by Professor Rhona Flin. I was given permission to run this survey by Dr George Crooks of Scottish Ambulance Service.
I am interested in paramedics' attitudes towards team work and safety when on a call. This questionnaire will be measuring non-technical skills such as attitudes towards communication, stress and fatigue, team work and situation awareness. These skills can play a major role in error management and can have an impact on effective team performance and consequently on patient safety.
I would kindly ask you to help me with my research, which will take approximately 15 minutes of your time. The questionnaire is anonymous and confidential so please take this opportunity to express your opinion and be frank and honest in your responses. No individuals will be identifiable in the research report. A full report will be sent to the organisation.
Thank you very much for your participation in this survey

Working patterns

Please indicate your level of agreement with the following statements relating to your work responding to 999 or Urgent calls and working with either a technician or another paramedic:

It is better to agree with other crew members than to voice a different opinion.
I let other crew members know when my workload is becoming excessive.
Before arriving at the scene, I consider potential problems which may occur and think about how I could solve them.
I am more likely to make errors in tense or hostile situations.
Good communication and crew co-ordination are as important as clinical skills.
I always assume that those that are more senior, or have more experience than me, are correct.
We should be aware of, and sensitive to, the personal problems of other crew members.
If I am interrupted while carrying out a procedure, I back up a few steps or start again.
The senior crew member should take control and make all decisions in emergency situations.
Please indicate your level of agreement with the following statements relating to your work responding to 999 or Urgent calls and working with either a technician or another paramedic:

My performance is affected by working with others less experienced or capable than me.
Even if I am in a hurry, I try to listen and not interrupt or “talk over” others.
The decisions or actions of the senior crew member should not be questioned by other crew members.
Even when tired, I perform effectively during critical phases of work.
Crew members understand their roles and responsibilities.
We regularly conduct debriefing after a call or shift.
My decision making is as good in emergencies as is it is routine situations.
Crew members should monitor each other for signs of stress or tiredness.
Supervisors who encourage suggestions from team members are weak leaders.
The senior crew member should verbalise plans and should be sure that the information is understood and acknowledged by other crew members.
Poor communication is the most common cause of reported errors.
Please indicate your level of agreement with the following statements relating to your work responding to 999 or Urgent calls and working with either a technician or another paramedic:

A truly professional paramedic can leave personal problems behind when at work.
I periodically review, check, and if necessary reassess the situation during a job
Personal conflicts between crew members can affect patient safety.
Crew members usually share information about possible risks and complications.
The patient’s condition always determines which crew member is the driver.
Crew members share the responsibility for prioritising activities in high workload situations.
If I perceive a problem with the management of a patient, I will speak up regardless of who might be affected
I always ask questions when I feel there is something I don’t understand.
My concentration is as good in the middle of the night as it is in the middle of the day.
Crew members usually share information about possible risks and complications.

Comparing yourself to other paramedics/ technicians of similar experience how would you rate your own skills?
