• Nomination of Beneficary Form
  The Nomination of Beneficary Form should be completed, ensuring that the proportion of benefits adds up to 100% if there is more than one beneficiary.
• Potential Dependent Nomination Form
  The Potential Dependent Nomination Form should be completed if there is anyone financially dependant upon you other than a spouse e.g. child or partner. Please see pages 13 & 14 of the handbook for details.
• Member Application Form
  Complete this form if you wish to join the scheme immediately on joining the University, or if you will not be automatically enrolled in the scheme 3 months after joining the University.
