Biography

Professor Gilbert graduated in medicine from Glasgow University in 1978. After completing house jobs, a year was spent as a research fellow in oncology under Professor Kenneth Calman, Glasgow, stimulating an interest in research in this area. Following general medical training and MRCP, she joined the radiological training programme in Aberdeen Royal Infirmary. She was appointed consultant radiologist in Aberdeen responsible for the National Breast Screening Programme for Grampian in 1989. Undertaking research in breast imaging and ovarian screening during that time she learnt research methodology and was appointed to the Chair in Radiology in 1996. Successful grants from SHEFC led to the purchase of a 1.5T MRI machine and a new building and office accommodation was put in place. She has built a team of research scientists working on MR related problems and a 3.0T MRI system was installed in August 2007 dedicated for clinical research. 

She is vice-chair of the Royal College of Radiologists Breast Group, member of the NCRI Breast Clinical Focus group, co-lead of the NCRI PET Research Development group and was member of the ACORRN international scientific advisory committee.  She sat on guideline development groups for breast imaging for SIGN and NICE. She was the Radiology Representative – Scottish Cancer Group (2001-2005) and member of the Cancer Research Portfolio Steering Group, Chief Scientist's Office (2003–). She was a member of the Commissioning Board for the Health Technology Assessment Panel (2003-2006) and Royal College of Radiologists Research Committee (1998-2002).  She is on the MRC Panel of Experts and is an Academic role model for the British Medical Association.

She referees for Radiology, the Breast and was assistant editor of Clinical Radiology (1997-2006).  She has been a National Panellist for Radiology (1995-2007) and FRCR Part I Examiner, Royal College of Radiologists (1997- 2002). She was joint chairman – Scottish Audit Gastric and Oesophageal Cancer Group (1996-2001) and chairman of the Computer Aided Diagnosis working party for the NHSBSP.

Research Interests

Breast imaging has remained a strong research focus. Interest in familial breast cancer led initially to a collaboration with the geneticists to identify women in the NHSBSP who had a strong family history. These women were recruited into the IBIS trial of Tamoxifen prevention, the FH01 study of the effect of annual mammographic screening and the high risk group were recruited into the MARIBS trial of MRI breast screening to detect early breast cancer. This has led to international collaborations with the Dutch and Canadians who have undertaken similar MR studies. Concern over the potential of readers to overlook early signs of breast cancer on mammograms led to an interest in Computer Aided Diagnosis. Following her review for the NHSBSP a retrospective evaluation of CAD in the NHSBSP was undertaken and the CRUK and the NHS funded a prospective multicentre randomised controlled trial to answer the question whether or not a single reader using CAD can perform to the same level as two readers in diagnosing breast cancer in a screening mammogram environment. This study has been completed and published in the New England Journal of Medicine. Further analysis of this data set is ongoing. Work is being undertaken on breast Tomosynthesis.

The use of dynamic contrast enhanced MRI to understand tumour processes during response to chemotherapy has been a longstanding interest initially using the standard software and now using sophisticated compartmental modelling techniques. Successful correlation of findings with PET in a series of patients undergoing neo-adjuvant chemotherapy has led to refinement of the model and the application of this technique to other tumour sites - rectal and cervical cancer. Molecular profiling is being undertaken at similar timepoints to image acquisition in the hope of identifying molecular processes to explain the imaging observations. 

Evaluation of technologies used in Radiology is an important area and successful work has been undertaken assessing the use of MRI in low back pain (HTA) and internal derangement of the knee (MRC) and in evaluating the place of endoscopic ultrasound in staging oesophageal cancer (HTA).

In collaboration with the bone programme we are developinging MRI techniques to measure muscle volume and changes in muscles during exercise; MRI is being used to assess changes in osteoarthritis in the hip and knee and also relative fat content in the medulla.
